Job Description: EHCP Coordinator
Responsibilities:
- Manage assessments and casework within the Special Educational Needs and Disabilities (SEND) Service, ensuring legal compliance while maintaining a child-centered, proactive, and responsive approach, considering parents’ perspectives.
1. EHCP coordinator – Pertemps Social Care
- Facilitate a person-centered, outcome-focused, multi-agency approach to assessment, planning, and support, ensuring children, young people, and parents are equal partners.
- Guide settings and parents toward appropriate provision at the SEN Support level, directing them to the Local Offer for information on available services.
- Support settings and parents in providing necessary information for EHC needs assessment requests, coordinating the 20-week process, and liaising with settings, parents, and professionals. Attend EHC plan review meetings, especially during educational transitions.
- Chair and attend annual reviews for cases with a high risk of placement breakdown or complex needs within targeted age groups.
- Utilize support from Team Leaders/Team Manager as needed.
- Ensure all communication with young people, parents, schools, settings, and agencies regarding SEN casework and general SEN issues is proactive, supportive, timely, and constructive to build and maintain confidence in the service.
Requirements:
- Current and valid Social Work England registration and pin.
- Experience working with Education, Health and Care Plans (EHCPs) or similar settings.
- Excellent communication and interpersonal skills.
- Ability to work independently and within a multidisciplinary team.
- Commitment to providing high-quality care.
- Access to a vehicle for community/home visits.
Hours and Contract:
- Flexible working hours, Monday-Friday availability required.
- 3-6 month contract with a strong possibility of extension based on performance and service needs.
