Role Overview
We are seeking a Care Assessor to join our growing team in Darwen. In this role, you will be responsible for assessing care needs, developing tailored care plans, and ensuring the highest quality of support for our clients. You will work closely with families, healthcare professionals, and our care team to maintain exceptional service standards.
Key Responsibilities
- Conduct new client assessments and create person-centered care plans.
- Complete regular care reviews to ensure the best outcomes for clients.
- Support and guide the care team in delivering high-quality care.
- Maintain strong relationships with clients, families, and healthcare professionals.
- Assist with general office administration.
- Participate in the on-call management rota.
Requirements
- NVQ Level 3 in Health & Social Care.
- Experience in a similar role within the care sector.
- Excellent communication and organizational skills.
- Full UK driving license and access to your own vehicle.
