Home Health Occupational Therapist
Your Role & Responsibilities
As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence by assessing their daily living and work-related skills and developing personalized therapeutic programs. Your work will support patients in their homes or other settings, helping them restore function, adapt to challenges, and improve their quality of life.

Evaluate patients’ functional abilities using diagnostic tests and assessments, assisting physicians in determining their level of function.

Collaborate with physicians and healthcare teams to develop and adjust individualized treatment plans.

Provide hands-on therapeutic treatment to improve strength, coordination, range of motion, and overall function, while also teaching compensation strategies when needed.

Continuously refine retraining programs to align with each patient’s behavioral and rehabilitation goals.

Work alongside vocational team members to create and implement vocationally oriented plans that meet patients’ needs and capabilities.

Monitor and report patient progress to physicians, clinical managers, rehabilitation staff, and family members.

Participate in care coordination and discharge planning, serving as a resource for patients and healthcare professionals.

Design community reintegration activities to help patients with physical recovery and psychological adjustments, coordinating with the interdisciplinary team.

Recommend and develop specialized adaptive equipment to enhance patients’ living or working environments and their rehabilitation journey.

Train caregivers, staff, and family members in assisting patients with essential daily activities such as dressing, feeding, grooming, and hygiene.

Supervise and evaluate the performance of Occupational Therapy Assistants and Home Health Aides in implementing therapy services.

Ensure timely and accurate documentation of patient care, interventions, and evaluations, while also properly recording and implementing physician orders.
Qualifications & Requirements

Degree in Occupational Therapy from an accredited program.

Minimum of six months of occupational therapy experience preferred; home health experience is a plus.

Active and unrestricted Occupational Therapist license.

Current CPR certification.

Strong organizational and communication skills.

Valid driver’s license, auto insurance, and reliable transportation.
Compensation
The salary range below reflects a good-faith estimate of starting base pay for full-time (40-hour workweek) employment. Actual pay may vary based on location, skills, experience, education, certifications, and other job-related factors.

Per Visit/Unit Rate: $49.00 – $69.00

Annual Base Pay: $77,200 – $106,200
