Key Responsibilities

  • Apply advanced clinical knowledge to identify, resolve, and document complex medication-related issues for a specific patient population.

  • Accurately interpret and evaluate prescriber orders for appropriateness, safety, and completeness.

  • Ensure safe medication preparation and dispensing while documenting clinical interventions and medication distribution discrepancies.

  • Support medication reconciliation and patient education on home regimens, collaborating with care teams to ensure coordinated discharge planning.

  • Monitor therapeutic outcomes and adverse effects, consulting with providers and documenting in the medical record.

  • Provide accurate, evidence-based drug information to providers, nursing staff, patients, and families, applying clinical literature to individual care plans.


Required Qualifications

  • Active Pharmacist license issued by the Oregon State Board of Pharmacy.

  • Completion of a residency program or equivalent experience; or board certification (e.g., BCPS, BCOPS).

  • Experience in inpatient hospital pharmacy serving diverse care settings, including acute, psychiatric, rehabilitation, long-term care, oncology, hospice, and more.

  • Demonstrated competencies in:

    • Safe, accurate medication therapy management and patient-centered care.

    • Age- and disease-specific therapy modifications.

    • Evidence-based decision-making using clinical literature.

    • Independent work with minimal supervision in high-paced environments.

    • Effective communication and teamwork across clinical and non-clinical teams.

    • Prioritization and critical thinking in emergent situations.

    • Use of EMRs, pharmacy databases, and other healthcare technologies.

    • Professional judgment, problem-solving, and compliance with clinical standards and policies.


Preferred Qualifications

  • PGY-2 Specialty Residency

  • BLS/ACLS certification

  • Immunization certification

  • Teaching certificate

  • Oncology pharmacy experience (inpatient and/or outpatient)

  • Proficiency in managing chemotherapy and supportive care therapies

  • Board certifications (e.g., BCPS, BCOPS)

  • State of Oregon Pharmacy Preceptor license


Compensation

Hourly Rate: $74.23 – $101.92
(Compensation based on experience, qualifications, and internal equity)
