About the Role:
As an entry-level Clinical Pharmacist, you’ll play a vital role in ensuring safe and appropriate pharmacy services for our home care patients. You’ll collaborate closely with the Nursing Department, monitor patient progress, and educate patients, caregivers, staff, and other healthcare professionals on the optimal use of medications.
Key Responsibilities:
- Assess patient suitability for home care services.
- Design, recommend, monitor, and evaluate individualized pharmacy care plans.
- Make recommendations for medication therapy adjustments and monitoring.
- Conduct nutritional assessments and provide appropriate recommendations.
- Modify care plans based on patient monitoring data.
- Educate Option Care staff, patients, and caregivers on medication use.
- Ensure seamless continuity of care across different settings.
- Prepare and dispense medications accurately, adhering to safety and quality standards.
- Manage medication administration and related equipment.
- Participate in medication-related incident reporting and equipment failure processes.
- Respond to drug information requests from healthcare providers and patients.

- Effectively utilize pharmacy support personnel.
- Maintain patient confidentiality and comply with all relevant regulations.
Qualifications:
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license in the relevant state.
- One of the following:
- At least 1 year of experience in sterile IV admixture procedures.
- Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency Program.
- Completion of an Advanced Pharmacy Practice Experience.
- Equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services.
- Strong customer service skills.
- Basic proficiency in Microsoft Word, Excel, and PowerPoint.
- Willingness to travel up to 5% for business purposes.
