As a Home Health Physical Therapist, You Will:

Design and administer tailored physical therapy treatment plans for patients suffering from various injuries, illnesses, and disabilities, aiming to help them achieve the highest level of physical function.

Conduct physical strength assessments to assist physicians in evaluating the patient’s condition, record findings, and develop treatment programs with measurable objectives.

Develop and implement rehabilitation programs in line with the physician’s treatment plan and the patient’s goals, making adjustments as necessary to maximize outcomes.

Collaborate with the physician and clinical team to gather additional patient information, and assist in refining treatment plans and the overall care strategy.
Required Experience/Skills

A degree from an accredited Physical Therapy Program (approved by the APTA).

A minimum of one year of physical therapy experience is preferred.

A valid and unrestricted Physical Therapy license.

Current CPR certification.

A valid driver’s license, auto insurance, and reliable transportation.

Pay Range: The compensation range for this position is between $85,400 and $117,500 per year, reflecting a good faith estimate based on a full-time (40 hours per week) schedule. The actual pay will vary depending on geographic location, skills, knowledge, experience, education, and certifications.
