Key Responsibilities

  • Provide hands-on care to residents, assisting with activities of daily living such as bathing, dressing, grooming, eating, and toileting to promote health and independence

  • Support mobility and functional care in alignment with each resident’s nursing care plan, including transfers, ambulation, positioning, range-of-motion exercises, and equipment use (e.g., Hoyer lift, whirlpool)

  • Build professional, respectful relationships with residents, their families, and fellow team members

  • Assist with routine clinical tasks including vital signs (TPRs, BP), weights, oxygen monitoring, specimen collection, and basic skin care

  • Follow infection control standards, including handling biohazard materials and implementing isolation protocols

  • Support care plans related to fall prevention, safety, behavior management, skin integrity, and nutrition/hydration needs

  • Collaborate with the nursing team to observe and report changes in resident condition, ensuring timely and appropriate care

  • Maintain a clean, organized environment in resident rooms and shared spaces

  • Follow protocols related to Resident Rights, HIPAA, abuse reporting, and emergency procedures

  • Encourage and facilitate resident participation in social and recreational activities

  • Accurately complete care documentation, including LNA flow sheets and data collection per the resident’s Care Plan


Qualifications

  • Active New Hampshire Licensed Nursing Assistant (LNA) License

  • Prior experience working with senior residents is highly preferred
