About the Job

We are seeking a dedicated and skilled Physical Therapist to join our team. In this role, you will design and implement individualized treatment plans aimed at restoring physical function, relieving pain, and preventing further physical challenges. You’ll perform comprehensive initial evaluations, develop appropriate diagnoses, and adjust treatment plans based on patient progress.

Key Responsibilities

  • Conduct thorough patient evaluations to identify physical challenges and establish treatment diagnoses.

  • Develop and execute personalized therapy programs to maximize patient outcomes.

  • Adjust treatments as needed based on patient response and progress.

  • Administer manual therapy techniques such as exercises, massage, or traction.

  • Educate patients and their families on at-home treatment procedures.

  • Collaborate with physicians and interdisciplinary teams to create and assess treatment plans.

  • Review medical records and physician referrals to determine appropriate interventions.

  • Accurately document patient progress, outcomes, and updates in charts or EMR systems.

  • Obtain informed consent for treatment plans.

  • Discharge patients when therapy goals are achieved and coordinate follow-up or referrals.

  • Supervise, mentor, and evaluate physical therapy students during clinical rotations.

Education & Professional Development

  • Serve as a clinical instructor for students (after one year of clinical experience, if applicable).

  • Support the education and training of students, even when not the primary clinical instructor.

  • Share evidence-based practices through presentations and in-services.

  • Attend institutional training sessions, staff meetings, and ongoing education programs.

Patient/Employee Safety

  • Actively participate in system-wide and departmental safety initiatives.

  • Maintain annual competency requirements and follow best practices for patient safety.

Operational Responsibilities

  • Conduct therapy evaluations, plan treatments, and implement care aligned with best practices.

  • Practice ethically, legally, and within state licensure regulations.

  • Accurately document treatments and participate in regular compliance and chart audits.

  • Meet or exceed departmental productivity expectations.

  • Display proper identification and follow institutional policies.

Qualifications

Education

  • Bachelor’s degree in Physical Therapy required; Master’s degree preferred.

Experience

  • Previous experience in a healthcare setting is preferred.

Licenses/Certifications

  • Active Physical Therapist license required.

  • Current CPR/BLS certification through the American Heart Association required.
