Join CenterWell Senior Primary Care: Lead with Purpose in Senior-Focused Care

Become a key member of Humana’s rapidly expanding Primary Care Organization, a leader in value-based, senior-focused care with over 175 centers nationwide (operating as CenterWell & Conviva). As a Primary Care Physician (PCP), you’ll lead a multidisciplinary team (including Advanced Practice Professionals, PharmDs, Care Coach Nurses, MAs, Behavioral Health Specialists, and more) in a patient-centered model that prioritizes quality and comprehensive well-being for seniors. This role allows you to see fewer patients, enabling more focused and impactful care.

Responsibilities:

  • Evaluate and treat patients according to established standards.
  • Maintain high-quality care and monitor patient outcomes using data and chart reviews.
  • Coordinate patient services (specialty referrals, hospital/SNF coordination, DME, home healthcare).
  • Act as a medical expert and active participant in daily care team huddles.
  • Support the Regional Medical Director and Center Administrator in fostering a collaborative environment.
  • Complete timely and accurate medical record documentation, working with quality-based coders.
  • Adhere to clinical leadership policies and protocols.
  • Collaborate with the RMD on quality, outcomes, policies, and records.
  • Participate in growth opportunities for center services.
  • Participate in the local primary care on-call program as needed.
  • Ensure compliance with licensing and accreditation requirements.
  • Dedicate 100% of your time to direct patient care and related administrative tasks.

Required Qualifications:

  • Bachelor’s degree.
  • 5+ years of technical experience.
  • Relevant state licensure.
  • Graduate of an accredited MD or DO program.
  • Preferred Internal Medicine specialty.
  • Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Successful completion of Humana/Senior Bridge’s Tuberculosis (TB) screening program (as this is a patient-facing role).
  • Passion for improving consumer experiences in healthcare.

Compensation: $203,400 – $299,500 per year (commensurate with location, experience, and qualifications), plus bonus potential.
