Position Summary

As a Clinical Pharmacist at Providence Centralia Hospital, you will play a vital role in promoting safe, effective, and cost-efficient medication therapy for our patients. This position is responsible for reviewing patient-specific medications and clinical data to ensure optimal therapeutic outcomes. Responsibilities include medication administration, clinical data monitoring, patient education, and serving as a trusted source of drug information for the healthcare team. You’ll also collaborate with clinical pharmacy leadership on quality improvement initiatives and opportunities for professional development.

Required Qualifications

  • Education:

    • Bachelor’s Degree in Pharmacy from an accredited School of Pharmacy

    • OR Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy

  • Licensure:

    • Active Washington State Pharmacist License (must be obtained prior to hire)
