Key Responsibilities

Clinical Practice

  • Collaborate with providers to personalize and optimize drug therapies, ensuring safety and cost-effectiveness.

  • Conduct thorough medication therapy reviews and ensure seamless care transitions.

  • Monitor and adjust drug regimens under collaborative drug therapy agreements, including lab test ordering and interpretation.

  • Authorize refills and assess patient adherence, interactions, efficacy, and safety.

  • Provide expert drug information to patients and clinical teams.

  • Support patient access to specialty medications by assisting with insurance requirements.

  • Proactively identify under-treated conditions and optimize therapy plans.

  • Recognize and address adherence barriers, accounting for cultural, emotional, socioeconomic, and physical factors.

  • Educate patients on disease and medication self-management.

  • Promote guideline-based prescribing through provider education and pattern reviews.

  • Manage therapy for anticoagulation, infectious disease, pain/palliative care, or other conditions, including patient education and long-term monitoring.

Education & Training

  • Serve as a preceptor for pharmacy residents and student pharmacists.

  • Educate medical residents, nursing staff, and students.

  • Present clinical topics at multidisciplinary meetings.

  • Deliver nursing in-services and didactic lectures through the University of Washington School of Pharmacy.

Research

  • Collaborate on research projects and drug studies.

  • Support investigational drug protocols.

  • Submit scholarly work for publication in peer-reviewed journals.

Practice Management

  • Develop and maintain pharmacy protocols and drug monitoring systems.

  • Stay current on clinical literature relevant to service lines.

  • Foster clear communication across departments and disciplines.

  • Contribute to quality assurance initiatives, including medication safety and cost-efficiency programs.

  • Conduct drug utilization reviews and cost-containment analysis as needed.

Dispensing

  • Provide dispensing services in infusion, retail, or oral chemotherapy pharmacies as required.


Qualifications

Required

  • PharmD or equivalent clinical experience (PGY-1 residency or 3+ years of relevant practice).

  • Active Pharmacist License in the State of Washington.

  • At least one year of experience in a hospital or clinical pharmacy setting.

Preferred

  • Completion of a PGY-2 Residency in Oncology or equivalent specialty experience.


Compensation & Benefits

  • Hourly Range: $70.40 – $117.33 (based on experience and qualifications)
