Your Impact:
As a Home Health Occupational Therapist, you’ll play a vital role in helping patients regain independence and improve their quality of life. You’ll evaluate and treat individuals recovering from illness, injury, or surgery, focusing on functional skills, activities of daily living, and home safety.
Responsibilities include:

Conducting thorough assessments of patient needs, including ADLs, fine motor coordination, and home safety evaluations

Developing and delivering personalized treatment plans in collaboration with physicians and the care team

Monitoring and documenting progress while adapting care plans as needed

Communicating patient responses and any changes in condition to the nurse supervisor and physician
-
Providing evidence-based care in alignment with state licensure regulations
Qualifications:

Active Occupational Therapy license in the state of Illinois

CPR certification
-
Valid driver’s license, auto insurance, and access to reliable transportation
