Wellbeing Link Worker (Full-Time, Permanent)

Salary: £27,350 per year
Hours: 36 hours per week
Location: Halton (with work across Cheshire & Merseyside)
Reports to: Services Manager

Wellbeing Enterprises is expanding its passionate team and currently hiring a Wellbeing Link Worker to support people referred by healthcare providers, community services, and self-referrals. About the Wellbeing Link Worker Role

As a Wellbeing Link Worker, you’ll offer one-to-one, personalised support to individuals, helping them take control of their health and wellbeing. You’ll work closely with people to explore ‘what matters to them,’ co-create Personalised Wellbeing Plans (PWPs), and connect them to local groups, services, and resources.

Key Responsibilities for a Wellbeing Link Worker

  • Provide tailored support via telephone, in community settings, and home visits

  • Build strong, trusting relationships with people from diverse backgrounds

  • Develop and manage Personalised Wellbeing Plans with each individual

  • Connect people to culturally appropriate community groups and VCFSE organisations

  • Collaborate with local partners to address social, practical, and emotional needs

  • Support initiatives to reduce loneliness, improve mental health, and promote healthier lifestyles
