Essential Duties and Responsibilities

Verify resident identity using wristbands or photo card files before administering treatments, serving meals, etc.

Use only authorized abbreviations for recording information.

Report any changes in the resident’s condition to the Nurse Supervisor/Charge Nurse promptly.

Document patient care clearly and descriptively on flow sheets, notes, and charts.

Report all accidents and incidents observed during the shift.
-
Safeguard resident health information and report any violations of confidentiality.

Report any unauthorized attempts to access the facility’s information system.

Perform only the nursing care procedures you are trained to do.

Ensure resident rooms are prepared for arrival, with made beds, name tags, and admission kits ready.

Greet and escort residents to their rooms, introducing them to roommates and staff as appropriate.

Make residents comfortable and manage their personal possessions, including inventory and storage.

Assist with packing and transferring personal items when residents move rooms or are discharged.

Transport residents as needed.

Help load and unload residents to and from vehicles when necessary.

Maintain a warm, positive environment that encourages comfort and care.

Regularly meet with nursing staff to identify and address service improvement areas.

Report complaints and grievances from residents.

Participate in nursing reports at the beginning of shifts.

Follow safety procedures for blood/body fluid exposure.
-
Perform housekeeping duties such as making beds and changing linens.

Measure and record residents’ vital signs (temperature, pulse, respiration), weight, and other required assessments.

Ensure residents who cannot call for help are checked frequently.

Answer resident calls promptly and check on personal care needs.

Assist residents with food identification and hydration needs.
-
Maintain clean water pitchers for residents.
-
Perform after-meal care (e.g., remove trays, clean residents).
-
Follow all assigned tasks according to policies and procedures.

Maintain communication and cooperation with other departments to meet residents’ needs.

Report any occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals according to policies.

Assist residents with personal care, such as bathing, dressing, and dental care.
Knowledge, Skills, and Abilities

Ability to read, analyze, and interpret business publications, technical documents, and government regulations.

Ability to write clear reports and business correspondence.
-
Strong problem-solving skills to address practical issues in variable situations.

Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.

Ability to occasionally lift and move up to 25 pounds.

Regular use of office equipment, such as computers, phones, and fax machines.

May assist in evacuating residents during emergencies.

Work environment typically has a low to moderate noise level.
Education & Experience
-
Minimum education requirement: 10th grade.
-
Must be a licensed Certified Nursing Assistant in accordance with state laws.
- CPR certification is preferred.
