Position Overview:
Assured Home Health, part of the LHC Group family, is currently seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide in-home care for patients in the Seattle, WA area. This role focuses on delivering high-quality nursing services under the guidance of the care plan, in close collaboration with an interdisciplinary healthcare team.
Key Responsibilities:

Deliver nursing services within the scope of practice as defined by Washington state regulations and consistent with the patient’s individualized care plan.

Provide in-home nursing care to assigned patients, strictly following the established plan of care without making modifications.

Maintain accurate and timely documentation by completing a detailed progress note for each visit and submitting it to the agency daily.

Actively participate in LHC Group’s in-service education programs to stay current with clinical best practices and policies.

Contribute to the agency’s Quality Assurance and Quality Improvement (QA/QI) initiatives to support continuous care enhancement and patient safety.
Qualifications & Requirements:

Licensure: Active and unrestricted Licensed Practical Nurse (LPN) license in the state of Washington.

Transportation: Reliable transportation with a valid driver’s license and current vehicle insurance—or access to dependable public transit.

Certifications: Current CPR (Cardiopulmonary Resuscitation) certification is required.
