Now Hiring: Licensed Practical Nurse (LPN)
Sun River Health is currently seeking a full-time Licensed Practical Nurse (LPN) to join our care team at one of our locations in the Hudson Valley, NYC, or Long Island.

Position Overview
The Licensed Practical Nurse (LPN) supports medical care delivery by providing nursing services to patients under the guidance of medical providers and in alignment with Sun River Health’s policies. This role includes a wide range of responsibilities including patient assessments, clinical documentation, and lab procedures, all performed in accordance with regulatory and organizational standards.

Key Responsibilities

  • Conduct and document patient interviews and clinical observations, including health history, vital signs, vision and hearing tests, and current medications.

  • Record patient data accurately in medical records, utilizing appropriate patient identifiers and documentation tools (e.g., PEAs, PhQ, Pain, Smoke Status, CDSS).

  • Perform and document in-house lab procedures and collect specimens for external testing.

  • Complete and maintain logs and assigned clinical tasks in accordance with organizational protocols.

  • Support care coordination by initiating follow-up actions as directed by the provider (e.g., encounters, labs, resolving non-billables).

  • Provide patient education in line with provider instructions and diagnoses; refer patients to the Registered Nurse when needed.

  • Assist with data collection under supervision—note: LPNs may not triage or interpret clinical data independently.

Required Qualifications

  • Education: High school diploma or GED (required); completion of a vocational or technical nursing program (preferred)

  • Experience: 0–1 year (required); 2–4 years (preferred)

  • Licensure/Certifications:

    • Current New York State LPN license

    • CPR certification

    • Infection Control certification

Compensation

  • Hourly Rate: $27.00 – $33.00

  • Job Type: Full-time
