Your Role as a Home Health LPN:

Deliver professional nursing care under the supervision of a Registered Nurse (RN) and in compliance with Florida’s Nurse Practice Act and CenterWell policies.

Provide skilled interventions to address acute, rehabilitative, and preventive healthcare needs using a holistic approach.

Implement therapeutic treatments, education, and training activities aligned with the patient’s care plan.

Collaborate with the RN Supervisor to update or revise the care plan as necessary.

Communicate verbal orders from physicians to the RN Supervisor when allowed by law and organizational policy.

Participate in patient care coordination and discharge planning to ensure a smooth transition and continuity of care.

Maintain accurate, timely documentation of all visits and interventions on the same day services are delivered.

Practice the highest standards of confidentiality and professionalism when handling sensitive patient information.

Ensure appropriate availability and use of supplies, materials, and equipment to safely carry out the plan of care.
Qualifications & Requirements:

Graduate of an accredited Licensed Practical Nursing (LPN) or Vocational Nursing program.

Active and unrestricted LPN license in the state of Florida.

Valid driver’s license, reliable transportation, and current auto insurance.

Current CPR certification.

Minimum two years of LPN experience, preferably in home health or hospice settings.
Scheduled Weekly Hours: 40
