Position Summary – Family/Internal Medicine Physician
Provide comprehensive primary care including diagnosis, treatment, care coordination, preventive services, and EHR management, following ACCESS’s quality, safety, and service standards.

Clinical Responsibilities
-
Perform patient histories, physical exams, diagnosis, and treatment
-
Manage EHR documentation, orders, and communication within 24-72 hours
-
Prescribe medications per Illinois law and clinical guidelines

Participate in on-call rotation and emergency response
-
Provide culturally competent care, patient education, and family involvement

Coordinate with care teams and ensure timely, quality patient management
Provider Development
-
Support staff training and certification
-
Participate in quality assurance and proficiency testing

Serve as a preceptor for trainees when applicable

Adhere to HRSA and FTCA regulations
Qualifications
-
M.D. or D.O. degree with completed residency and Board Certification in primary care

Illinois medical license, DEA registration, and current CME
-
Minimum two years outpatient primary care experience preferred

EHR proficiency, EPIC preferred
-
Strong cultural competency and interpersonal skills
