About the Role

We are seeking a dedicated and compassionate Community Mental Health Nurse (Band 5) to join our multidisciplinary team. In this essential role, you will provide high-quality care that meets the mental, physical, spiritual, psychological, and social needs of service users in the community, with a focus on dignity, respect, and patient empowerment.

You will be central in delivering Clozapine monitoring services, administering IM injections, performing venepuncture, conducting physical health checks, and working closely with patients, carers, and the wider mental health team to ensure recovery-focused care.

Key Responsibilities

Clozapine Clinic Support

  • Lead day-to-day functioning of the Clozapine Clinic with support from the nursing team, pharmacists, and consultants.

  • Monitor and report Clozapine side effects in line with clinical protocols.

  • Perform IM injections, venepuncture, and interpret physical health monitoring results appropriately.

Clinical Care & Patient Support

  • Conduct risk assessments, care planning, and reviews in accordance with Trust policy.

  • Respond appropriately to concerns related to a patient’s mental health or wellbeing.

  • Assist patients in self-management and everyday skills, including medication management, budgeting, shopping, and accessing support services.

Team & Multidisciplinary Coordination

  • Actively contribute to MDT meetings and participate in information sharing and decision-making.

  • Liaise with welfare, housing, employment, and substance misuse services on behalf of patients.

  • Support the safe use and storage of medical equipment and maintain stock control.

Professional Practice & Development

  • Maintain high standards of documentation and clinical communication in line with NMC guidelines.

  • Stay up to date with national and local policy developments and evidence-based practice.

  • Engage in supervision, mentorship, and student nurse assessment.

Quality, Safety, and Policy Compliance

  • Participate in audits (e.g., hand hygiene audits) and ensure compliance with Trust safeguarding protocols.

  • Promote a safe clinical environment and uphold patient dignity and respect at all times.

  • Recognise and appropriately manage challenging behaviour, safeguarding risks, and psychosocial concerns.

What We’re Looking For

Skills & Experience:

  • Registered Mental Health Nurse (RMN) with valid NMC registration

  • Experience in community mental health services (preferred)

  • Knowledge of Clozapine management, mental health law, safeguarding, and CPA

  • Skilled in venepuncture, IM injections, and physical health monitoring

  • Excellent communication, interpersonal, and organisational skills
