We are looking for a Program Manager to join our team.

Responsibilities include but are not limited to:

  • Provides support, direction and assistance to all staff and clients under programs (Assisted Living Services, Home First, Home at Last, Nursing);
  • Collaborates with CCD Managers to improve processes, problem solve issues, develop best practices, create synergies and ensures a holistic client experience;
  • Identifies gaps in service delivery and care coordination while providing recommendations for efficiencies and quality improvement in conjunction with the Vice President as needed;
  • Plans, implements and evaluates the PSW-supported services, in consultation with the Vice President to meet client needs;
  • Conducts client visits for assessment and assist in developing care plans in conjunction with Care Coordinator;
  • Monitors Human Capital and Corporate Services metrics, and reviews with staff to ensure program and MSSA targets are achieved;
  • Oversees hiring and program orientation process, plans for staffing needs, and participates in recruitment cycle in consultation with Vice President and Human Capital;
  • Upholds confidentiality, safety, and accuracy of client information and files in CCD database(s);
  • Participates in policy development and program-supporting procedures, guidelines, practices and implementation;
  • Manages fiscal responsibilities through the development of local service plans and budgets, administration of financial resources within the approved budgets;
  • Develops formal annual recommendations for program improvement and innovation to the Vice President that align with the organizational Strategic Plan;
  • Evaluates the program annually with the Vice President for efficiency and quality improvements;
  • Establishes and maintains appropriate community and organizational contacts;
  • Identifies opportunities to promote and spread awareness for all the CCD programs/opportunities in the community;
  • Provides backup to CCD Managers in their absence and participates in Management On-Call rotation.

Qualifications required:

  • Post secondary education in Nursing, Bachelor of Science in Nursing is an asset
  • Registered Practical Nurse / Registered Nurse in good standing with the College of Nurses of Ontario is required
  • Minimum of three (3) years’ experience in a Management role
  • Good knowledge of the principles and practices of personal support services, nursing, and scheduling
  • Experience in client assessment and staff relations
  • Intermediate level computer skills (MS Office and Alayacare)
  • Proven knowledge and skills with client records management, electronic medical records, and external reporting
  • Experience working in a non-profit and Community Supports is an asset
  • Able to provide leadership, support, and critical analysis in stressful, clinical and emotional situations
  • Self-directed and highly motivated with excellent interpersonal and communication skills (written and verbal)
  • Excellent organizational and coordination skills, and the ability to prioritize multiple competing workload demands
  • Experience with geriatric mental health, cognitive impairment and dementia
  • A proven ability to liaise with referral sources and other community agencies
  • First Aid Certification with HCP Level CPR training
  • Valid “G” Ontario driver’s license and access to a safe and reliable vehicle
  • Satisfactory Police Vulnerable Sector Check
  • Sensitivity to adults with needs related to aging, physical and/or mental health
  • Current immunization record (TB test completed within the past 12 months) is required upon hire
  • Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Starting at $85,000 annually; 3 weeks’ vacation per year of work (pro-rated to hours worked); 9 paid (including stat) holidays; health, insurance and pension benefits when eligible.

* Pay will be commensurate based on education and experience
