Essential Responsibilities

Provide direct patient care under a collaborative practice agreement, establishing and managing individualized therapeutic medication regimens.

Serve as a clinical resource by responding to provider inquiries and offering evidence-based medication guidance.

Support clinic staff in facilitating medication access, including processing prior authorizations, appeals, manufacturer assistance program enrollments, third-party grants, and copay assistance programs.

Deliver medication-related education to patients, healthcare teams, and community members.

Participate in the development and implementation of clinical guidelines, protocols, and policies.

Lead or contribute to quality improvement initiatives and projects aimed at advancing clinical pharmacy practice.

Serve as a preceptor for PGY1 and PGY2 pharmacy residents as well as pharmacy students.

Perform other related duties as assigned.
Clinic-Specific Roles
Anticoagulation Clinic
The Ambulatory Anticoagulation Pharmacist is responsible for managing anticoagulation therapy for assigned patients. In addition, the pharmacist supports the management of other cardiovascular conditions such as hypertension, dyslipidemia, cardiomyopathies, cardiovascular risk reduction, and heart failure by providing clinical consultations and medication access assistance.
Heart Failure Clinic
The Heart Failure Pharmacist manages the pharmacotherapy of patients with heart failure. They may also support related cardiovascular conditions, including anticoagulation therapy, hypertension, dyslipidemia, cardiomyopathies, and preventive cardiology, while serving as a clinical resource and aiding in medication access.
Flormann Clinic
The Pharmacist at the Flormann Clinic primarily manages patients with endocrinologic conditions. They may also assist in pulmonology and rheumatology care by offering clinical guidance and ensuring medication access.
Qualifications
Required:

Education: Bachelor of Science in Pharmacy

Licensure/Certification:

Active Pharmacist license with the South Dakota Board of Pharmacy

Basic Life Support (BLS) certification from the American Heart Association (AHA)

Education: Doctor of Pharmacy (PharmD)

Experience:

Completion of a PGY1 pharmacy practice residency and a minimum of three years of clinical pharmacy experience in an ambulatory care setting; OR
-
Completion of a PGY2 residency in Ambulatory Care (or specialized focus such as Cardiology); OR
-
Five or more years of clinical pharmacy experience in an ambulatory care environment

Certification: Board certification is strongly encouraged
