Job Summary
The Ambulatory Care Pharmacist delivers clinical medication management services in an outpatient setting. Working under a collaborative practice agreement, the pharmacist partners with physicians and advanced practice providers to optimize therapeutic outcomes for selected patients.

Job Description

Essential Duties:

  • Provide direct patient care under a collaborative practice agreement, developing patient-specific therapeutic medication plans.

  • Respond to clinical inquiries from clinic providers as needed.

  • Support clinic staff with medication access processes, including prior authorizations, appeals, patient enrollment in manufacturer assistance programs, third-party grants, and copay assistance.

  • Educate healthcare teams, patients, and the community on medication-related topics.

  • Participate in developing and implementing clinical guidelines and policies.

  • Lead quality improvement projects and initiatives to advance pharmacy practice.

  • Serve as a preceptor for PGY1 and PGY2 pharmacy residents and pharmacy students.

  • Perform other duties as assigned.

Specialized Clinic Roles:

  • Anticoagulation Clinic: Manage anticoagulation therapy for clinic patients and assist with other cardiovascular conditions such as hypertension, dyslipidemia, cardiomyopathies, cardiovascular risk reduction, and heart failure. Provide clinical support and medication access assistance.

  • Heart Failure Clinic: Oversee medication therapy for heart failure patients, support management of cardiovascular conditions including hypertension, dyslipidemia, cardiomyopathies, cardiovascular risk reduction, and anticoagulation therapy. Provide clinical guidance and assist with medication access.

  • Flormann Clinic: Primarily manage endocrinology medication needs of patients, with additional support in pulmonology and rheumatology. Provide clinical expertise and help facilitate medication access.

Requirements:

Education:

  • Bachelor’s degree in Pharmacy required.

  • Doctorate in Pharmacy preferred.

Certifications:

  • Licensed Pharmacist by the South Dakota Board of Pharmacy.

  • Basic Life Support (BLS) certification from the American Heart Association.

Experience:

  • Completion of a PGY1 pharmacy residency plus at least three years of clinical pharmacy experience in ambulatory care, or

  • Completion of an Ambulatory Care PGY2 residency (or a focused specialty such as cardiology), or

  • Five years of clinical pharmacy experience in ambulatory care.

Certifications:

  • Board certification is encouraged.

Physical Requirements:

  • Light work involving exerting up to 20 pounds of force occasionally, up to 10 pounds frequently, or negligible force constantly to move objects.
