Key Responsibilities

  • Serve as an in-clinic resource, answering provider questions related to formulary options, dosing, side effects, drug interactions, and more.

  • Actively demonstrate the value of pharmacy collaboration within physician practices.

  • Maintain and manage a patient schedule for services such as medication counseling, immunizations, medication administration, and pillbox refills.

  • Track and follow up with patients to ensure medication efficacy, adherence, and safety, while communicating care plans with providers.

  • Support refill coordination and maintain compliant, auditable proof-of-delivery documentation.

  • Prepare and submit prior authorizations, and manage appeals or denials as appropriate.

  • Address prescriptions for limited distribution drugs or those requiring out-of-network handling.

  • Advocate for patients financially by utilizing charitable foundations, manufacturer programs, and public assistance options to enhance medication access.

  • Navigate and document in EMR systems and specialty therapy platforms efficiently and accurately.

  • Collaborate in the development and expansion of ambulatory pharmacy services by participating in project teams and pharmacy navigator committees.

  • Cross-train in ambulatory pharmacy operations and assist with staffing when needed.


Qualifications

Education:

  • Doctor of Pharmacy (PharmD) degree required

  • PGY1 residency or prior experience in ambulatory care and diabetes management strongly preferred

Licensure:

  • Unrestricted Pharmacist license in Pennsylvania (including immunization certification) required

  • Out-of-state pharmacists must obtain PA licensure within 90 days of receiving an offer

Experience:

  • Familiarity with the 340B Drug Pricing Program is a plus

  • Experience with Epic or similar EMR systems

  • Understanding of retail pharmacy processes and prescription claims adjudication

Technical Skills:

  • Proficient in Microsoft Office applications (Word, Excel, PowerPoint)

  • Ability to analyze and interpret clinical data, lab values, and patient notes effectively

  • Comfortable working with diverse and complex medical conditions
