Job Summary

The Care Manager serves as a trusted advisor to families, building strong client relationships and ensuring exceptional care delivery. This role requires passion for serving older adults and individuals with disabilities, the ability to identify family needs, and expertise in providing tailored solutions. Candidates should have clinical or geriatric care management experience and strong interpersonal skills to foster client trust and loyalty.

Key Responsibilities

In-Home Visits & Assessments

  • Conduct initial in-home assessments and develop personalized care plans.

  • Build rapport with families to support conversion from prospect to client.

  • Review services and pricing with clients to ensure smooth onboarding.

Ongoing Client Support

  • Serve as the primary point of contact for assigned clients.

  • Maintain up-to-date client records and communications in ClearCare.

  • Collaborate with the care coordination team to ensure caregiver fit.

Professional Care Management

  • Provide additional services including benefits administration, trust and financial planning, family counseling, and coordination of medical resources.

Positive Health Outcomes

  • Educate caregivers on client diagnoses and care needs.

  • Track health-related KPIs, including readmission rates, recovery trends, and hospitalizations.

  • Ensure proper caregiver training and introductions before placement.

Qualifications

  • Bachelor’s degree in social sciences, social work, or related field.

  • 1+ year experience in a clinical setting or client relationship management.

  • Experience in geriatric care management preferred (ALCA certification a plus).

  • Willingness to travel (one week per month per region).

  • Strong interpersonal, organizational, and problem-solving skills.

  • Experience in a sales-driven environment preferred.

Technical Skills

  • Proficient in Microsoft Office 365.

  • Experience maintaining electronic records (ClearCare experience is a plus).
