Key Responsibilities:

Interpret, verify, dispense, and monitor medications prescribed by providers.

Evaluate and communicate comprehensive medication information to patients, families, and healthcare professionals.

Perform daily medication monitoring, distribution, and acquisition activities.

Manage medication reconciliation, anticoagulation therapy dosing and monitoring, pharmacokinetic evaluations, and renal dose adjustments.

Provide medication-related consultations and support to Essentia Health staff.

Support departmental operations, including billing, recordkeeping, audits, staff training, and precepting pharmacy students, interns, and residents.

Maintain regulatory compliance and uphold quality standards across all pharmacy practices.
Education Qualifications:

Graduate of an accredited college of pharmacy (BS or PharmD degree required).

Completion of an ASHP-accredited residency program or a minimum of three years of hospital pharmacy experience preferred.
Licensure/Certification Requirements:

Licensed as a Registered Pharmacist by the Board of Pharmacy in the state of practice.

Pharmacists working at SMPHM Union sites must be licensed in both Minnesota and Wisconsin within one year of hire if working 0.4 FTE or greater.

Candidates not licensed at hire must hold a valid Pharmacy Intern license and obtain Registered Pharmacist licensure within 90 days of hire.

Ongoing continuing education and licensure renewal per state requirements.
