Key Responsibilities:
Clinical Care & Patient Interaction

Conduct comprehensive patient interviews to collect medical, medication, and immunization histories.

Identify and address drug allergies, contraindications, and potential interactions.

Provide direct pharmaceutical care by adjusting drug regimens in accordance with CDTM protocols.
-
Order and interpret relevant laboratory tests and perform clinical assessments (e.g., A1C, vital signs).

Monitor therapeutic outcomes and adverse reactions; modify treatment plans as needed.

Deliver clear and compassionate education to patients and caregivers on medication use and adherence.

Assist with navigating insurance and access barriers, including prior authorizations and patient assistance programs.
Documentation & Communication

Maintain thorough documentation in the electronic medical record (EMR), including therapy adjustments and patient progress notes.

Communicate therapy updates to other members of the care team and coordinate follow-up as necessary.

Present clinical insights to healthcare teams and support staff training through in-services and educational programs.
Operational Leadership & Program Development

Serve as an operational expert on pharmacy workflow, policies, and ambulatory services.

Collaborate with clinic and pharmacy teams to streamline processes and optimize resource use.

Develop and track outcome metrics, quality indicators, and performance goals for pharmacy-driven programs.

Identify strategic opportunities to expand clinical services, such as immunizations and adherence initiatives.
Compliance & Professional Standards

Practice in accordance with NYC Health + Hospitals protocols, NYS regulations, and collaborative agreements.

Ensure ongoing compliance with licensing, certification, and regulatory requirements.

Maintain strong knowledge of system and facility formularies, prior authorization processes, and pharmacy systems.
Education & Mentorship

Contribute to the education and mentorship of pharmacy residents and students.

Support interdisciplinary learning through presentations, grand rounds, and preceptorships.
Minimum Qualifications:
-
Valid New York State pharmacist license and current registration.
-
Certification to practice under a CDTM agreement by the NYS Education Department.
Preferred Qualifications:
-
Proficiency in Microsoft Office Suite, including Visio, Project, Word, and Excel.

Experience with EPIC EMR and Willow Ambulatory systems.

Immunization and BLS (Basic Life Support) certification.

Strong communication, leadership, and interpersonal skills.
