Key Responsibilities

Clinical Care

  • Conduct comprehensive patient interviews to obtain medical, medication, immunization, and health histories.

  • Screen for drug allergies and interactions, and identify medications contraindicated in specific disease states.

  • Manage, monitor, and adjust medication regimens based on therapeutic outcomes, following CDTM protocols.

  • Order and interpret lab and physical assessments (e.g., A1C, microalbumin, foot exams) as authorized.

  • Evaluate lab results and modify therapy accordingly to improve clinical outcomes and safety.

  • Perform medication reconciliation and resolve discrepancies to enhance treatment effectiveness.

  • Educate patients and caregivers on medication usage, changes, and the importance of adherence.

  • Communicate therapy updates clearly with patients, families, and other healthcare providers.

  • Document interventions, progress notes, adverse drug reactions (ADRs), and medication errors in EMR.

  • Support patients in accessing prescriptions, resolving coverage barriers, and coordinating with assistance programs.

Operational & Strategic Impact

  • Collaborate with ambulatory care teams to establish and standardize CDTM protocols.

  • Set measurable patient care goals and track outcomes to support continuous quality improvement.

  • Promote services such as immunizations and additional clinical offerings aligned with strategic priorities.

  • Monitor and report key performance indicators (KPIs) and quality metrics to enhance patient satisfaction and safety.

  • Act as a resource for care teams, removing barriers to therapy and streamlining coordination.

Education & Mentorship

  • Provide education to healthcare staff through grand rounds, in-services, and clinical presentations.

  • Precept pharmacy students and residents during clinical rotations, supporting training and professional growth.

Compliance & Regulation

  • Practice within the guidelines of CDTM protocols, ensuring compliance with institutional policies and state/federal laws.

  • Maintain active pharmacy licensure, CDTM certification, and other regulatory requirements.


Qualifications

Minimum Requirements:

  • Active and valid New York State Pharmacist License and Registration.

  • Certification from the New York State Education Department to practice under CDTM agreements.


Preferred Qualifications

  • Experience with EPIC EMR and Willow Ambulatory systems.

  • Immunization certification and current BLS certification.

  • Strong written and verbal communication skills.

  • Advanced proficiency in Microsoft Office tools (Visio, Excel, Word, Project).

  • Strong interpersonal and collaborative skills.
