About the Job

Position Overview: Centralized Order Entry Pharmacist (Work From Home, Night Shift)
Schedule: 7 days on / 7 days off (70 hours per pay period)
Licensure: Valid pharmacist license in at least one of the following states is required – VA, MO, NH, KY, or IN.

Key Responsibilities:

  • Review and evaluate medication orders to ensure appropriate drug therapy

  • Accurately process orders in a timely manner, ensuring compliance with protocols

  • Identify and prevent drug interactions, duplications, and contraindications

  • Monitor IV drugs for compatibility, dosage, and administration rate

  • Investigate and report adverse drug reactions and medication events

  • Provide clinical interventions and documentation

  • Ensure age-appropriate medication use across the lifespan—from neonates to geriatric patients

Qualifications:

  • Bachelor’s degree in Pharmacy required; PharmD preferred

  • Active pharmacist license in VA, MO, NH, KY, or IN (at least one is mandatory)

  • Minimum 1 year of hospital pharmacy experience preferred

  • Familiarity with Meditech is a plus
