Position Summary
The primary responsibility of this role is to provide daily nursing care and support to assigned residents, following each individual’s care plan and under the direction of nursing supervisors. This position plays a key role in ensuring residents receive compassionate, high-quality care in a safe and comfortable environment.
Essential Duties and Responsibilities

Identify residents accurately using wristbands or photo cards before administering care, treatments, or meals.

Use only approved facility abbreviations when documenting information.

Report any changes in a resident’s condition to the Nurse Supervisor or Charge Nurse promptly.

Document all care and observations clearly and accurately in charts, flow sheets, and notes.

Report all incidents and accidents immediately as they occur.
-
Protect resident privacy by maintaining confidentiality and reporting any suspected violations to the Administrator.

Report any unauthorized access attempts to the facility’s information systems.
-
Perform only those nursing care tasks you are trained and authorized to carry out.

Prepare rooms for new admissions, including making beds and ensuring supplies are available.

Welcome and escort new residents to their rooms, introduce them to roommates and staff, and help them settle in.

Inventory and label residents’ personal items, and assist with packing and storing belongings during transfers or discharges.

Transport residents within the facility and assist with vehicle transfers as needed.

Promote a warm, respectful, and calming atmosphere throughout the unit.

Participate in regular team meetings to address concerns and improve resident care.
-
Report any resident complaints or grievances immediately.

Attend and participate in nursing shift reports as directed.
-
Follow all infection control and exposure protocols regarding blood and bodily fluids.

Assist with bed-making and provide extra bedding upon request.

Record residents’ temperature, pulse, and respiration (TPR) as directed; weigh and measure residents when required.

Frequently check on residents who are unable to call for help.
-
Respond promptly to resident calls and ensure personal care needs are met regularly.
-
Assist visually impaired residents with meal identification and arrangement.

Track and report changes in residents’ food and fluid intake; assist with after-meal care.

Maintain clean, filled water pitchers within reach of residents at all times.

Complete assigned duties in accordance with policies, procedures, and supervisory instructions.

Work collaboratively with all facility departments to ensure high-quality resident care.

Notify the facility in advance of any tardiness or absences.

Follow proper procedures to report occupational exposure or hazardous incidents.

Provide support with daily living activities, including hygiene, grooming, dressing, and bathing.

Keep residents dry and clean by changing clothing and linens as needed.

Assist with preparations for medical appointments and testing.
-
Help move, lift, position, and transport residents safely.
-
Support rehabilitative and restorative care efforts as directed.
-
Monitor for skin breakdown and pressure ulcers, and report findings immediately.

Document and report unknown injuries or skin tears.
-
Observe and document the condition of disoriented or comatose residents.

Carry out special treatments as instructed.

Immediately notify supervisory staff if a resident is missing or has left the facility.

Use only equipment you are trained to operate; report any malfunctions promptly.

Maintain strict confidentiality of all resident information, including protected health data.

Report all allegations of abuse or misappropriation of property immediately.

Respect a resident’s right to refuse treatment and report such instances to the supervisor.
Knowledge, Skills, and Abilities

Ability to read, understand, and interpret professional literature, regulations, and written procedures.

Capable of writing detailed reports, business correspondence, and procedural documentation.

Effective verbal communication skills for interactions with staff and supervisors.

Proficient in basic mathematical concepts such as fractions, percentages, and ratios.

Strong problem-solving skills and the ability to work with minimal standardization.

Able to understand and follow written, oral, diagrammatic, or scheduled instructions.

Must be able to occasionally lift and/or move up to 25 pounds.
Education and Experience Requirements

Minimum of a 10th-grade education.
-
Must hold current certification as a Certified Nursing Assistant (CNA) per state regulations.
-
CPR certification preferred.
