As a member of the Hospice Team at Bluegrass Care Navigators (BCN), you’ll have the opportunity to deliver compassionate, patient-centered care to individuals facing terminal illnesses. Our focus is on enhancing comfort and quality of life, rather than curative treatments.

You’ll provide one-on-one care in the setting that patients call home—whether that’s a private residence, assisted living facility, nursing home, or hospital. For patients with more complex needs, we also offer care in our Hospice Care Centers, where they receive expert attention and support.

About the Role

The Certified Nursing Assistant is a vital part of the hospice care team, working under the supervision of a Registered Nurse to deliver personal care services that enhance patient comfort and safety. You’ll monitor and report patient conditions and ensure care is documented accurately. This role requires empathy, attention to detail, and the ability to work independently in a variety of care environments.


Qualifications

  • Minimum 18 years of age with a high school diploma or equivalent

  • State Registered Nurse Aide certification through the State Board of Nursing

  • Completion of an approved CNA training program

  • At least 1 year of experience in a hospital or nursing home (preferred)

  • Strong observational, documentation, and communication skills

  • Ability to follow written care instructions

  • Empathetic, self-directed, and professional with good personal hygiene

  • Familiarity with the philosophy and goals of hospice care

  • Valid driver’s license, reliable insured vehicle, and auto liability insurance of at least $100,000/$300,000 bodily injury coverage

  • Willingness to comply with BCN’s health and vaccination policies
