Key Responsibilities:

  • Support patients with self-administered medications by assisting with bottle caps, reading medication labels, verifying prescribed doses, and observing administration.
  • Accurately take and document vital signs, including temperature, pulse, and blood pressure, and promptly report abnormal results to the supervisor.
  • Help patients maintain personal hygiene through bathing, grooming, skin care, oral care, nail/foot care, and related tasks.
  • Assist with feeding, applying basic nutrition principles, recording food and fluid intake, and ensuring safe positioning for meals.
  • Support patients with toileting needs, including bedpan/urinal use, catheter bag changes, and ostomy care as appropriate.
  • Assist patients with mobility tasks such as repositioning, transfers, ambulation, and exercises according to the care plan and CenterWell policies.
  • Report patient condition changes and document all care provided accurately and thoroughly to ensure continuity of care.
  • Promote infection control best practices and maintain a clean, safe, and comfortable environment.
  • Assist with light household duties such as laundry, bed making, meal preparation, light housekeeping, and shopping when needed and authorized.

Required Experience and Skills:

  • High school diploma or equivalent.
  • Completion of a state-approved Certified Nursing Assistant (CNA) or Home Health Aide (HHA) program within the last 24 months.
  • Active CNA/HHA certification meeting state requirements.
  • Valid driver’s license, reliable transportation, and auto insurance.
  • Must be in good standing on the HHA Registry (if applicable) and eligible to work with a Medicare-certified agency.
  • Minimum of one year of relevant experience within the past two years in a hospital, nursing home, or home health/hospice setting.

Scheduled Hours:
40 hours per week

Compensation:
Estimated annual salary: $41,300 – $48,200
Pay may vary based on location, experience, and qualifications.
