Clinical Pharmacist – Oncology Focus at Fred Hutch
Join Fred Hutchinson Cancer Center in Seattle, the only NCI-designated cancer center in Washington, and contribute to our mission of eliminating cancer and infectious diseases. As a Clinical Pharmacist, you’ll be an integral part of our patient care team, collaborating with providers, nurses, and research staff to optimize pharmacotherapeutic outcomes for our adult oncology patients.
Responsibilities:
- Individualize drug therapies and ensure safe, effective, and economical patient care.
- Conduct comprehensive medication reviews and manage transitions of care.
- Monitor and modify drug regimens, including ordering and interpreting lab tests (when referred).
- Provide refill authorizations and act as a drug information resource.
- Ensure patient access to specialty medications by providing clinical information to insurers.
- Identify and optimize drug therapy for under-treated patients.
- Address barriers to medication adherence and educate patients on self-management.
- Review prescribing patterns and provide targeted education to clinic staff.
- May manage specialized therapies (anticoagulation, infectious disease, pain/palliative care) under collaborative agreements.
- Precept pharmacy residents and students, and provide education to other healthcare professionals.
- Participate in collaborative research projects and contribute to publications.
- Contribute to practice management, including policy development, drug monitoring, and quality assurance.
- Perform dispensing pharmacist duties as needed (oral chemo, infusion, or retail pharmacy).
Required Qualifications:
- PGY1 residency completion OR 3 years of relevant experience.
- Current State of Washington Pharmacist License.
- One year of experience in hospital pharmacy practice.
Preferred Qualifications:
- PGY2 Oncology residency.
