Oncology Formulary Management and Utilization:

Manage the oncology formulary by evaluating and incorporating new medications into the system.

Assess and determine the most appropriate site of care for patients undergoing oncology treatment, including inpatient, outpatient, or regional facilities.

Supervise the review process for off-label, non-formulary, or non-standard uses of antineoplastic agents.

Encourage the appropriate, standardized, and guideline-based use of antineoplastic medications across all El Camino Health (ECH) locations when applicable.

Evaluate provider requests for exception orders and review supporting literature to guide the prior authorization process where needed.

Collaborate with physicians and other healthcare professionals to ensure the safe and effective use of oncology medications.
Collaboration with Pharmacy Informatics Teams:

Work closely with pharmacy informatics teams to help develop and update Electronic Health Records (EHR) resources.

Ensure EHR resources are aligned with current oncology medication guidelines and best practices to support their safe and effective use.
Education:

Coordinate and provide education, training, competency assessments, and performance improvement for pharmacists, pharmacy residents, and other healthcare professionals on the proper use of antineoplastics, best practices, and the latest oncology pharmacy developments.

Partner with other disciplines to develop and implement best practices for antineoplastic utilization across the institution.
Quality and Performance Improvement:

Contribute to the Antineoplastic Stewardship Program by collecting data and metrics on antineoplastic usage in both inpatient and outpatient settings, and report findings to relevant stakeholders.

Monitor antineoplastic utilization to identify areas for improvement and implement necessary changes.

Build and maintain databases and dashboards to support the Antineoplastic Stewardship Program as required.

Collaborate with the Department of Quality and Patient Safety to assess, mitigate, and disseminate safety initiatives related to antineoplastics.

Stay updated on the latest advancements in oncology pharmacy to ensure pharmacy services align with current standards of care.

Contribute to the creation and maintenance of policies and procedures supporting the safe use of antineoplastic medications across all ECH locations to enhance patient outcomes through safe, effective, and cost-effective medication therapies.
Qualifications:
-
Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited school of pharmacy.

One year of accredited PGY1 pharmacy residency.

One year of PGY2 Pharmacy Residency in Oncology/Hematology or Board-Certified Oncology Pharmacist (BCOP) certification with at least 2 years of oncology experience, including pharmacy regulatory compliance and compiling Medication Use Evaluation (MUE) or quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.
License/Certification/Registration Requirements:

Valid California State Pharmacist License with no disciplinary actions preventing the practice of pharmacy.

Valid California driver’s license.
