As a Clinical Pharmacy Specialist, you’ll deliver advanced pharmaceutical care within your specialty area by collaborating with physicians, educating healthcare staff, contributing to system-wide initiatives, and mentoring pharmacy residents and students. Your efforts will help ensure safe, effective, and evidence-based medication therapy throughout the health system.
Key Responsibilities

Deliver specialized clinical pharmacy services including drug therapy monitoring, consultations, and medication education.

Participate in daily rounding with physicians and care teams to ensure optimal medication management aligned with institutional guidelines.

Evaluate and report adverse drug reactions; support medication safety initiatives.

Contribute to the Medication Safety and Therapeutics (MS&T) Committee by assessing formulary relevance and assisting with medication policy development.

Serve as a resource to pharmacists and healthcare providers for complex medication-related issues.

Precept pharmacy residents and students, and provide ongoing clinical education for pharmacy staff.

Maintain detailed documentation of clinical activities.

Engage in system-level committees and workgroups within your area of clinical focus.
Additional Duties

Ensure compliance with all institutional policies and professional standards.

Maintain confidentiality and protect patient health information (PHI) in accordance with regulations.
Qualifications
Education:
-
Doctor of Pharmacy (PharmD) with completion of an ASHP-accredited residency program, or
-
3–5 years of clinical pharmacy experience in a relevant practice area (preferred)
Licensure:
-
Must hold an active Registered Pharmacist (R.Ph.) license in the State of Ohio
Preferred Experience & Skills:

Experience with clinical pharmacology, pharmacokinetics, drug interactions, and cost evaluations

Strong verbal and written communication abilities

Proficiency in Microsoft Office applications (Word, Excel, database management)
Physical Requirements

Frequent standing, walking, lifting, and handling up to 50 lbs
