Job Responsibilities

Assess Patient Suitability for Home Care
- Gather and organize patient-specific information to evaluate eligibility for home care.
- Assess medical conditions and prescribed therapies based on Option Care policies to determine suitability.
- Define clear outcome goals for home care treatment plans.

Develop and Monitor Pharmacy Care Plans
- Design, recommend, and oversee individualized pharmacy care plans for home care patients.
- Suggest modifications to medication therapy or monitoring parameters when necessary.
- Identify and implement appropriate monitoring measures for each patient.
- Conduct nutritional assessments and make appropriate recommendations.

Adjust Treatment Plans Based on Patient Data
- Modify pharmacy care plans based on monitoring data and patient-specific details.
- Utilize pharmacokinetic principles and assess patient laboratory values for effective medication management.

Provide Medication Education
- Educate Option Care staff, healthcare professionals, patients, and caregivers on medication use.
- Offer counseling using effective patient education techniques.

Ensure Continuity of Care
- Systematically communicate key patient information between home care and other healthcare settings.
- Prepare and dispense medications following Option Care protocols.
- Calibrate equipment accurately and ensure proper medication concentration, stability, and storage.
- Follow safety and quality assurance guidelines, ensuring compliance with labeling policies.

Adhere to Regulatory and Safety Standards
- Inspect final medications before dispensing and verify authorization to dispense.
- Comply with state and federal laws, including DEA regulations and USP standards.
- Maintain a perpetual inventory system for controlled substances and prevent medication diversion.

Manage Medication Administration Equipment
- Operate, maintain, and troubleshoot medication administration devices.
- Train others on proper use and maintenance of equipment.
- Participate in medication-related incident reporting and equipment failure assessments.
- Document and report discrepancies in controlled medication inventories.

Provide Drug Information Support
- Respond to drug-related inquiries from healthcare providers and patients.
- Research and formulate evidence-based responses to medication inquiries.
- Stay current with continuing education (CEU) to maintain professional competency and meet state-mandated requirements.

Handle Investigational Drug Products
- Manage investigational drug products according to regulations and company policies.

Optimize Pharmacy Support Personnel
- Effectively allocate tasks and manage workflow to ensure timely completion.
- Oversee and verify the accuracy of work performed by pharmacy support staff.

Ensure Compliance and Confidentiality
- Adhere to all accreditation, legal, regulatory, and safety requirements.
- Maintain confidentiality of patient information and proprietary company data.
Basic Education and Experience Requirements
- Graduate of an accredited School of Pharmacy.
- Active, current pharmacy license issued by the State Board of Pharmacy.
- One of the following qualifications:
- At least one year of experience in sterile IV admixture procedures.
- Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
- Completion of an ASHP-accredited Pharmacy Residency Program.
- Completion of an Advanced Pharmacy Practice Experience through an accredited school of pharmacy.
- Equivalent experience approved by the Vice President (VP) of Pharmacy or Senior Vice President (SVP) of Clinical Services.
Basic Qualifications
- Experience providing customer service to both internal and external stakeholders, ensuring high-quality service and customer satisfaction.
- Proficiency in Microsoft Office applications:
- Word – Basic skills such as document formatting, text alignment, spell check, track changes, and printing.
- Excel – Basic functions including inserting rows/columns, formatting cells, aligning text, renaming worksheets, and using charts.
- PowerPoint – Basic functions such as inserting slides, editing text, rearranging content, and running presentations.
