Clinical Pharmacist – Partnership HealthPlan of California
Overview
We are seeking a Clinical Pharmacist to review Treatment Authorization Requests (TARs) and Coverage Determination Forms (CDFs), ensuring safe, effective, and cost-efficient drug therapy. This role involves communicating with prescribers and pharmacies, managing formularies, and contributing to Pharmacy & Therapeutics (P&T) meetings.
Key Responsibilities

Evaluate and process TARs for medication approvals.

Oversee pharmacy technicians in TAR/CDF reviews, ensuring compliance with prior authorization guidelines.

Collaborate with prescribers and pharmacies to provide formulary alternatives and guidance on approvals.

Participate in formulary management, drug utilization reviews, and authorization criteria development.

Recommend updates to PHC’s drug formularies.

Contribute to projects, in-service training, academic detailing, and pharmacy student preceptorship.

Support cross-departmental initiatives requiring clinical pharmacy expertise.

Perform other duties as assigned.
Qualifications
Education & Experience:

Bachelor’s degree in Pharmacy.

Minimum two years of experience in managed care, hospital, or corporate pharmacy.
Licenses & Skills:

Valid California Pharmacist License.

Knowledge of Medi-Cal pharmacy services.

Familiarity with local medical and pharmacy communities preferred.

Understanding of treatment guidelines and online medical/pharmacy resources.

Valid CA driver’s license and auto insurance for work-related travel.
