Pharmacist – Home Care & Infusion Services

Job Responsibilities

As a Pharmacist in Home Care & Infusion Services, you will play a vital role in ensuring safe, effective, and personalized medication management for patients receiving care at home. Your responsibilities will include:

  • Assessing Patient Suitability for Home Care: Reviewing physician referrals, medical records, and patient conditions to determine their eligibility for home-based pharmaceutical care. Ensuring treatment plans align with Option Care Health policies and patient prognosis.

  • Developing and Monitoring Pharmacy Care Plans: Creating tailored pharmacotherapy plans and adjusting treatments as necessary based on patient progress, laboratory values, and clinical data.

  • Providing Patient Education: Counseling patients and caregivers on medication use, safety protocols, and proper administration techniques to promote adherence and optimal outcomes.

  • Ensuring Medication Safety and Compliance: Preparing and dispensing medications following USP standards and state/federal regulations, including controlled substance guidelines. Ensuring accurate labeling, storage, and monitoring for potential drug interactions.

  • Maintaining Equipment and Inventory: Managing the use, maintenance, and troubleshooting of medication administration devices while ensuring inventory compliance, particularly with Schedule II narcotics and controlled substances.

  • Facilitating Continuity of Care: Communicating patient-specific medication needs with healthcare teams and ensuring seamless transitions between home care and other healthcare settings.

  • Adhering to Regulatory and Ethical Standards: Upholding HIPAA, accreditation, and legal requirements to maintain patient confidentiality and quality care standards.


Qualifications & Skills

Education & Licensing:

  • Graduate of an accredited School of Pharmacy.

  • Active Pharmacist License issued by the State Board of Pharmacy.

Experience (One of the Following Required):

  • 1+ year of experience in sterile IV admixture procedures.

  • Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM).

  • Completion of an ASHP-accredited Pharmacy Residency Program.

  • Completion of an Advanced Pharmacy Practice Experience (APPE) as part of a pharmacy school curriculum.

  • Equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services.

Technical Skills:

  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).

  • Strong organizational and customer service skills.

  • Ability to analyze clinical data and recommend effective pharmacotherapy adjustments.
