Job Responsibilities:

  • Deliver comprehensive pharmaceutical care to hospitalized patients, including medication therapy management and providing drug information support.

  • Conduct thorough medication reconciliations to ensure accuracy and patient safety throughout the care process.

  • Accurately prepare and dispense medications in alignment with physician orders and in compliance with all applicable state and federal regulations.

  • Monitor patient drug regimens for effectiveness, safety, and potential drug interactions, making clinical recommendations as needed.

  • Ensure compliance with USP 797 and USP 800 standards in sterile compounding areas, including IV medication preparation.

  • Collaborate closely with physicians, nurses, and other healthcare providers to optimize therapeutic outcomes.

  • Educate patients and healthcare staff on appropriate medication use and safety protocols.

  • Handle the preparation and dispensing of chemotherapy medications, including providing counseling to patients undergoing treatment.

  • Maintain accurate records of all medications dispensed and treatment plans for regulatory compliance and quality assurance.


Qualifications:

  • Valid and active New Jersey State Pharmacist License.

  • Basic Life Support (BLS) certification.

  • IV and Chemotherapy Certification required.

  • Hands-on experience in sterile compounding practices following USP 797/800 guidelines.

  • Proven background in inpatient pharmacy settings and conducting medication reconciliation.

  • Registered Pharmacist (RPh) credential required.
