About the Job
Beacon Health System is seeking a skilled and motivated Clinical Pharmacist to join our pharmacy team. In this role, you will contribute to the development and maintenance of pharmacy services that prioritize pharmaceutical care, patient safety, and clinical excellence. You’ll play a critical part in order verification, medication dispensing, clinical decision support, and patient counseling while demonstrating a strong commitment to providing exceptional customer service.

Key Responsibilities
Order Verification

Review all medication orders for accuracy, compatibility, dosing, and appropriateness for patients of all ages (pediatric, neonatal, geriatric).

Clarify or reject incorrect orders and follow up with appropriate documentation and communication.

Verify IV and oral medication orders using established protocols prior to nursing barcode scanning.

Monitor unverified orders to ensure prioritization and clinical follow-up.

Suggest formulary substitutions when necessary and collaborate with physicians for alternatives.
Dispensing & Compounding

Compound IV admixtures, TPNs, chemotherapy, and specialty solutions using aseptic techniques in compliance with USP 797 guidelines.

Supervise IV room operations and train new staff on aseptic technique and safety protocols.

Ensure timely preparation and labeling of STAT, NOW, and routine orders.

Maintain controlled substance compliance, Omnicell cabinet stocking, repackaging, and barcoding accuracy.

Monitor inventory and assist in efficient pharmacy stock management.
Clinical Services

Provide pharmacokinetic monitoring, TPN dosing, anticoagulation services, and antibiotic stewardship.

Document clinical notes in the EMR to ensure effective communication with the healthcare team.

Conduct patient counseling and education when required.

Lead or participate in hospital committees, in-services, and clinical projects.

Support ambulatory clinics, anticoagulation clinic, CHF clinic, and decentralized care areas.
Leadership & Support Functions

Assist in developing departmental policies, procedures, and power-plan reviews.

Supervise and mentor technicians and pharmacy externs, providing constructive feedback and training.

Contribute to special projects and regulatory compliance initiatives.

Maintain active participation in departmental and organizational meetings.
Education & Experience
-
Required: Bachelor’s degree in Pharmacy or PharmD from an accredited institution, or foreign pharmacy equivalency exam completion.

Licensure: Registered Pharmacist in the State of Indiana (or eligibility).

Preferred: Completion of a hospital pharmacy residency or equivalent experience.
Knowledge & Skills

Strong knowledge of pharmacy laws, antibiotic stewardship, anticoagulation, and parenteral nutrition principles.

Excellent verbal and written communication skills for effective collaboration with healthcare professionals.

Ability to work efficiently in high-paced environments while prioritizing patient safety.
