Responsibilities
The primary responsibility of this role is to assess, design, and manage patient-specific pharmacy care plans for individuals receiving home care. This includes evaluating patients’ medical conditions, prescribed medications, and ensuring the safety and efficacy of medication therapy in a home care setting. The pharmacist will:

  • Evaluate patient suitability for home care, assessing their medical condition, prescribed therapies, and prognosis in alignment with Option Care policies.

  • Design, recommend, and evaluate personalized pharmacy care plans, modifying therapy or monitoring parameters when necessary to optimize patient outcomes.

  • Identify and resolve medication-related issues by gathering and analyzing patient-specific information and data, including lab values and nutritional assessments.

  • Educate home care patients, caregivers, and Option Care staff on medication use, providing counseling and using effective patient education techniques.

  • Ensure continuous pharmaceutical care through coordination between home and other patient-care settings, facilitating communication and accurate medication dispensing.

  • Prepare medications in compliance with Option Care policies and procedures, ensuring proper labeling, storage, and quality assurance practices.

  • Manage medication administration equipment, offering advice on its maintenance and troubleshooting when necessary.

  • Participate in medication-related incident management and provide timely responses to drug information requests from healthcare providers and patients.

  • Oversee the use of investigational drug products, and manage pharmacy support personnel, prioritizing and organizing workloads effectively to meet operational needs.

  • Comply with legal, regulatory, and safety standards in all activities, maintaining confidentiality and adhering to ethical guidelines.

Supervisory Responsibilities
This position does not include direct supervisory responsibilities.

Basic Education and Experience Requirements

  • Graduate of an accredited School of Pharmacy.

  • Current, active pharmacy license issued by the State Board of Pharmacy.

  • One year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), or completion of an ASHP-accredited Pharmacy Residency Program, Advanced Pharmacy Practice Experience, or equivalent experience.

Basic Qualifications

  • Experience providing customer service to both internal and external customers, meeting quality standards and evaluating customer satisfaction.

  • Basic proficiency with Microsoft Word.

Preferred Qualifications and Interests

  • Completion of an ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience associated with an accredited pharmacy school.

  • Experience in home care, hospital settings, or clean room environments.

Management Requirements

  • At least one year of experience in indirect management, including assisting with team member development, training, and assignment of work.
