Position Overview: Clinical Pharmacist

Key Responsibilities

As a Clinical Pharmacist, you will ensure the safe, effective, and evidence-based use of medications throughout our health system. Your responsibilities will include:

  • Medication Review & Verification: Evaluate all medication orders across patient age groups for accuracy, appropriateness, and clinical effectiveness.

  • Therapeutic Monitoring: Monitor and assess drug regimens to identify side effects, adverse reactions, toxicities, interactions, and therapeutic effectiveness.

  • Collaborative Consultation: Partner with physicians, nurses, and other healthcare professionals to optimize medication therapy and resolve drug-related issues.

  • Pharmacist Interventions: Provide clinical recommendations for drug selection, dosage modifications, and treatment adjustments to improve patient outcomes.

  • Documentation: Maintain accurate patient medication profiles within the pharmacy information system and record pharmacist interventions and therapy recommendations.

  • Medication Preparation: Verify and, when needed, prepare medications using correct aseptic techniques and protocols.

  • Patient Engagement: Collect and assess medication histories on select patients, and relay pertinent information to the healthcare team.

  • Targeted Drug Programs: Participate in drug utilization reviews and monitor specific drug therapies based on institutional protocols.


Qualifications

Required:

  • Active and unrestricted Registered Pharmacist (RPh) license issued by the Texas State Board of Pharmacy

  • Completion of a 20-hour ACPE-accredited sterile compounding course in aseptic technique
