Company Overview:

Monogram Health is seeking a dedicated and experienced Clinical Pharmacist to support our medication therapy management (MTM) program for patients across Florida. In this fully remote role, the Clinical Pharmacist will play a vital part in improving medication outcomes for patients with chronic kidney and polychronic conditions. This includes performing comprehensive medication reviews, developing actionable drug therapy plans, and working collaboratively with internal care teams and external healthcare providers to ensure safe, effective, and evidence-based medication use.


Key Responsibilities:

  • Conduct and document comprehensive medication reviews (CMRs) in alignment with Monogram’s productivity and quality standards.

  • Deliver clinically meaningful interventions that support adherence to evidence-based medication guidelines by providers and patients.

  • Review patient care reports, clinical documentation, and medical records to inform medication action plans and quality initiatives.

  • Serve as a subject matter expert on medication management for Monogram’s field-based clinical teams (PODs).

  • Provide training to Monogram clinical staff to ensure a consistent, high-quality MTM program.

  • Track medication use patterns and stay current with clinical best practices for treating kidney disease and related chronic conditions.

  • Participate in interdisciplinary care team meetings and ensure timely distribution of CMR documentation to providers.

  • Implement processes to optimize medication use in line with patient and organizational needs.

  • Support pharmacy leadership in delivering outstanding, personalized medication management services.

Qualifications:

Required:

  • Doctor of Pharmacy (PharmD) or Bachelor’s degree in Pharmacy.

  • Active pharmacist license and residency in the state of Florida.

  • Minimum of 3 years of clinical or managed care pharmacy experience.

Preferred:

  • Completion of a PGY1 residency in ambulatory care, managed care, or a related clinical field.

  • Board certification through BPS (e.g., pharmacotherapy, geriatrics, ambulatory care).

  • MTM board certification through NBMTM or completion of MTM certificate training (e.g., APhA, Powerpak).

  • Experience supporting MTM programs within a value-based or integrated care setting.
