Position Summary:

Monogram Health is seeking a Clinical Pharmacist to join our growing Medication Therapy Management (MTM) program. This remote position plays a vital role in optimizing medication use and outcomes for members with complex kidney-related and chronic conditions. The Clinical Pharmacist will conduct comprehensive medication reviews (CMRs), develop individualized drug therapy action plans, and collaborate closely with Monogram care teams, nephrologists, and other healthcare providers to enhance patient care.


Key Responsibilities:

  • Complete concise, patient-specific comprehensive medication reviews (CMRs) in alignment with Monogram’s productivity and quality standards.

  • Identify and drive meaningful medication interventions that promote evidence-based prescribing and improve patient and provider adherence.

  • Review clinical documentation, including care reports and medical records, to support therapy planning and outreach efforts.

  • Collaborate with Monogram’s clinical field teams (PODs) to integrate MTM strategies into daily care operations.

  • Serve as a subject matter resource for internal clinicians on pharmacotherapy and MTM protocols.

  • Support MTM training initiatives to build consistency and competency across clinical staff.

  • Track and analyze medication usage trends among Monogram’s patient populations.

  • Actively participate in interdisciplinary care team meetings and ensure timely communication of medication recommendations to providers.

  • Implement medication optimization approaches that meet the needs of patients and organizational partners.

  • Contribute to MTM program development and support pharmacy leadership with clinical initiatives as needed.

Required Qualifications:

  • Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD)

  • Active and unrestricted pharmacist license in New Jersey

  • Minimum of 3 years of clinical experience, preferably in a managed care, ambulatory, or clinical practice setting

  • Must currently reside in New Jersey


Preferred Qualifications:

  • PGY1 residency in ambulatory care, managed care, or a related clinical discipline

  • Board certification (BPS) in pharmacotherapy, geriatrics, ambulatory care, or similar

  • MTM Board Certification (NBMTM) or MTM certificate from APhA, PowerPak, or equivalent
