As a Clinical Pharmacist at Monogram, working remotely within New Jersey, you will play a key role in advancing patient care by developing and implementing our comprehensive Medication Therapy Management (MTM) program. Your responsibilities will include performing thorough written medication reconciliations and creating individualized drug therapy action plans for all members. You will ensure clear communication of these plans to Monogram’s clinical team and external providers like nephrologists and primary care physicians. A crucial aspect of your work will be conducting telephonic consultations with medical providers to recommend medication adjustments and providing direct support to members by addressing their medication use concerns. You will work in a highly collaborative environment with nurse care managers, social workers, and operations personnel to integrate MTM effectively into Monogram’s patient care model and to oversee the proper execution of MTM policies by our field clinicians.
Desired Qualifications (Highly Valued):
While not mandatory, candidates with the following qualifications will be given strong consideration:
- Residency Training: Completion of a PGY1 (Post-Graduate Year One) residency in ambulatory care, managed care, or a related clinical field.
- Board Certifications (BPS): Board certification from the Board of Pharmacy Specialties (BPS) in Pharmacotherapy (BCPS), Geriatrics (BCGP), Ambulatory Care (BCACP), or a related clinical field.
- MTM Certification (NBMTM): Board certification in Medication Therapy Management from the National Board of Medication Therapy Management (NBMTM).
- MTM Training: Completion of an MTM certificate training program from the American Pharmacists Association (APhA), Powerpak, or a similar accredited program.
