Responsibilities

  • Identify, resolve, and document complex therapeutic issues, demonstrating advanced expertise in a specialized patient population.
  • Interpret and assess prescriber orders for accuracy, completeness, and appropriateness, documenting clinical decisions and medication distribution issues that require follow-up or ongoing evaluation.
  • Verify the accuracy and appropriateness of medications prepared and dispensed from the pharmacy.
  • Assist with medication reconciliation, educate patients on their home medication regimens, and coordinate discharge medication plans with the interdisciplinary care team. Document these activities in the patient’s medical record.
  • Monitor therapeutic responses and potential adverse effects of medications to guide drug therapy, consulting with providers as necessary and documenting interventions in the patient’s medical record.
  • Appropriately report adverse drug reactions, medication errors, and drug defects.
  • Provide accurate drug information to prescribers, nurses, patients, and their families, independently reviewing and interpreting medical literature to apply findings to specific patient care situations.

Required Qualifications

  • Current Pharmacist license issued by the Oregon State Board of Pharmacy.
  • Completion of a Pharmacy Residency or equivalent experience, or Specialist Certification (BCPS, BCOPS, etc.).
  • Experience in inpatient hospital pharmacy practice, with the ability to meet pharmaceutical care needs in various patient care settings, including acute care, psychiatry, rehabilitation, skilled nursing, long-term care, obstetrics, oncology, chemical dependency, HIV, home care, and hospice.
  • Demonstrated ability to:
    • Apply pharmaceutical care principles safely and accurately.
    • Modify drug therapy based on age and disease-related factors.
    • Evaluate medical literature effectively.
    • Work independently with minimal supervision.
    • Manage multiple tasks and frequent interruptions.
    • Communicate effectively both formally and informally, including delivering presentations.
    • Lead projects and collaborate professionally within a healthcare team.
    • Prioritize tasks and respond quickly to emergent situations.
    • Maintain attention to detail and exercise sound professional judgment.
    • Utilize key computer-related skills, including electronic medical records (EMR), word processing, spreadsheets, medical literature, and medication databases.

Preferred Qualifications

  • PGY-2 Specialty Residency.
  • BLS/ACLS certification.
  • Vaccination certification.
  • Teaching Certificate.
  • Experience in inpatient or outpatient oncology pharmacy practice.
  • Oncology-related drug therapy management experience, including chemotherapy and adjunctive medications.
  • Board Certifications (BCPS, BCOPS, etc.).
  • State of Oregon Pharmacy Preceptor licensure.
