Clinical Pharmacist – OHSU
Pay Range: $74.23 – $101.92/hour (commensurate with experience)
Overview
Join OHSU’s dedicated pharmacy team, delivering safe, effective, and personalized medication therapy to diverse patient populations—from neonates to the elderly. As a Clinical Pharmacist, you will play an integral role in optimizing medication use, supporting transitions of care, and educating patients and care teams across inpatient and outpatient settings.
Key Responsibilities

Ensure accurate and appropriate dispensing of medications

Proactively manage complex medication therapies and collaborate with care teams

Support transitions of care: med reconciliation, discharge planning, and patient counseling

Monitor therapeutic outcomes and adverse effects, documenting and communicating effectively

Serve as a drug information resource for providers, patients, and families

Rotate across areas including Research Pharmacy and Hematology/Oncology units
Required Qualifications

Oregon Pharmacist License
-
PGY-1 Residency or equivalent experience; relevant board certification preferred

Strong clinical judgment, communication, and interdisciplinary teamwork skills

Proficiency in EMR systems and drug databases
Preferred Qualifications

PGY-2 Specialty Residency (Oncology preferred)

BLS/ACLS, immunization certification, and teaching certificate

Experience with oncology or specialized drug therapy management
-
Oregon Pharmacy Preceptor license
