Position Overview
Senior Doc is seeking a knowledgeable and dedicated Clinical Pharmacist to join our team on a contracted per diem basis. This remote position focuses on providing expert medication management for seniors, especially those living with dementia. The ideal candidate will support healthcare providers in evaluating and optimizing medication regimens, with a particular emphasis on high-risk drugs and de-prescribing strategies.
Key Responsibilities

Collaborate with healthcare providers to assess and optimize medication therapy for seniors and dementia patients.

Identify and recommend de-prescribing opportunities to reduce medication burden and enhance patient safety.

Develop personalized treatment plans based on comprehensive medication reviews and patient history.

Perform medication reconciliation and address discrepancies to ensure continuity of care.

Educate patients, caregivers, and providers on safe medication use, potential side effects, and alternative therapies.

Document clinical interventions and communicate findings clearly with the interdisciplinary care team.
Qualifications

Doctor of Pharmacy (Pharm.D.) from an accredited institution.

Active pharmacist license in the state of California.

Minimum of 2 years of clinical pharmacy experience, preferably with a focus on geriatrics and dementia care.

In-depth knowledge of geriatric pharmacotherapy, de-prescribing principles, and cognitive impairment management.
