Position Summary
The Clinical Pharmacist plays a critical role in the preparation and dispensing of parenteral and enteral medications, ensuring accuracy and safety in every step. This position involves active monitoring of patient drug therapies and providing expert drug information to healthcare professionals. The Clinical Pharmacist also oversees pharmacy support personnel and may assume leadership responsibilities in the absence of the Pharmacy Director to ensure smooth departmental operations.
Qualifications
Education & Licensure

Bachelor of Science in Pharmacy or Doctor of Pharmacy (Pharm.D.) degree required

Active license to practice pharmacy in the State of Texas (required)

Completion of all mandated continuing education requirements
Experience
-
Minimum of two (2) years of hospital pharmacy experience preferred

At least twenty (20) hours of training in sterile IV admixture preparation
-
Familiarity with pharmacy automation systems, medication order entry tools, and inventory management processes

Proficient in generating and interpreting reports and clinical documentation

Key Responsibilities
Clinical & Operational Duties

Accurately review, verify, and process medication orders
-
Screen for drug interactions, dosage accuracy, and therapeutic appropriateness

Monitor for contraindications, duplications, and potential adverse drug reactions

Prepare and dispense a variety of medications, including IV admixtures, chemotherapy agents, and parenteral nutrition solutions

Maintain accurate medication profiles and reconcile with administration records

Perform medication histories and participate in medication reconciliation efforts
Compliance & Documentation

Maintain complete and accurate records of controlled substances, inventory logs, and compliance reports
-
Ensure strict adherence to applicable federal and state regulations and internal policies

Conduct scheduled audits of controlled substances and uphold secure storage practices

Manage medication recalls and document actions taken in accordance with protocols
Collaboration & Leadership

Supervise and provide direction to pharmacy technicians and support staff

Offer clinical consultation and drug therapy guidance to medical and nursing teams

Recommend cost-effective and evidence-based therapeutic alternatives

Participate in interdisciplinary rounds, clinical evaluations, and continuous quality improvement initiatives

Mentor and train pharmacy interns and students as a preceptor
Additional Responsibilities

Temporarily fulfill Pharmacy Director duties during their absence

Support formulary adherence and contribute to therapeutic substitution strategies

Generate medication fill reports and review culture and sensitivity data to guide therapy

Promote the safe, efficient, and cost-effective use of pharmaceuticals across the organization

Age-Specific Competency
The Clinical Pharmacist must demonstrate understanding of the unique developmental and healthcare needs of patients across the lifespan. This includes the ability to adjust medication management strategies and communication styles appropriately based on patient age and condition.
