Position Summary:
VCU Health is seeking a dedicated and knowledgeable Clinical Pharmacist to support our Medical/Respiratory ICU (MRICU) team. This role ensures the integrity, safety, and clinical effectiveness of medication use processes for patients across all age groups—neonates, pediatrics, adolescents, adults, and geriatrics. The Clinical Pharmacist will work collaboratively within a multidisciplinary team to optimize therapeutic outcomes for critically ill patients.


Key Responsibilities:

  • Participate in interdisciplinary patient care rounds, providing expert pharmacotherapy recommendations.

  • Monitor, screen, and verify medication orders for correct dosage, route, formulation, and schedule.

  • Resolve medication-related problems and clarify questionable orders with prescribers.

  • Develop and adjust individualized medication treatment plans based on clinical assessment.

  • Counsel patients on the proper use and safety of their medications.

  • Supervise and evaluate Pharmacy Interns, Certified Pharmacy Technicians, and Pharmacy Residents in accordance with state regulations and departmental policies.

  • Educate and mentor pharmacy students and residents.

  • Maintain up-to-date, accurate records and ensure compliance with regulatory and institutional standards.

  • Designate and oversee pharmacy technician assignments and support staff performance evaluations.

  • Support the safe and efficient preparation, dispensing, and delivery of medications.

  • Address equipment issues and operational challenges as they arise.

  • Foster a collaborative and inclusive team culture.


Licensure & Certification Requirements:

  • Required at Hire: Licensure by the Virginia State Board of Pharmacy (must be obtained within 90 days of employment if not already licensed)

  • Required for Continued Employment: Active Virginia Pharmacist license


Education Requirements:

  • Required:

    • Bachelor of Science in Pharmacy from an accredited academic institution

  • Preferred:

    • Doctor of Pharmacy (Pharm.D.)

    • Completion of ASHP-accredited PGY1 and/or PGY2 residency programs


Experience:

  • Required: None

  • Preferred:

    • At least one (1) year of clinical pharmacy experience in a similar setting or completion of pharmacy residency training

    • Familiarity with advanced medication distribution systems and clinical pharmacy services


Independent Duties:

  • Independently review and verify drug orders for accuracy and clinical appropriateness

  • Adjust drug therapy as needed to achieve optimal outcomes

  • Serve as a resource for complex pharmacotherapy decisions

  • Assign and direct support staff and evaluate their performance

  • Communicate effectively with interdisciplinary team members to coordinate patient care


Additional Requirements:

  • Availability to work rotating shifts, including evenings, nights, weekends, and holidays

  • Must serve a broad range of patient populations across all age groups
