Summary

The Clinical Pharmacist Specialist plays a vital role in both direct and indirect patient care, focusing on education, medication safety, performance improvement, and consultation. This position is responsible for delivering both operational and clinical pharmacy services to a diverse patient population. Required Qualifications:

  • Bachelor of Science in Pharmacy
  • Illinois Pharmacist License
  • A minimum of four years of Clinical Pharmacy experience OR completion of a PGY1 residency within the past seven years, including Clinical/Specialty Pharmacy experience
  • Proficiency in intravenous admixture services, IV drug therapy, and chemotherapy
  • Strong ability to collaborate with healthcare teams to provide safe and effective drug therapy
  • Excellent problem-solving skills, with the ability to work independently and efficiently

Preferred Qualifications:

  • BLS Certification
  • ACLS Certification (required upon hire if not already obtained)
  • Board Certification from the Board of Pharmaceutical Specialties
  • Doctor of Pharmacy (PharmD) degree with residency completion
  • Experience and strong knowledge in specialty drug therapies

Key Responsibilities:

  • Order verification, discharge counseling, and healthcare professional education
  • Review and verify Epic physician orders for accuracy, dosing, drug interactions, administration routes, and therapeutic effectiveness
  • Clarify prescriptions through direct, timely communication with prescribers
  • Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff
  • Document clinical interventions in line with pharmacy department protocols
  • Monitor pharmacy reports daily, including pharmacokinetics and patient-specific drug reports
  • Conduct monthly nursing unit inspections to ensure proper medication storage
  • Supervise pharmacy technicians, ensuring timely medication preparation and delivery
  • Coordinate medication ordering, preparation, dispensing, and administration systems
  • Implement Pharmacy & Therapeutics Committee policies on drug use and formulary decisions
  • Participate in Adverse Drug Reaction (ADR) and medication error reporting programs
  • Complete clinical and operational projects (e.g., Pyxis inventory optimization, process improvement initiatives)
  • Precept PGY1 and PGY2 pharmacy students and residents in both clinical and operational areas
  • Evaluate and interpret medical and drug literature to assess and compare treatment options
  • Implement medication safety initiatives based on departmental needs and research findings
  • Develop operational and clinical projects aligned with current departmental priorities
  • Serve as a liaison for interdisciplinary research and collaborative studies
  • Evaluate and monitor ambulatory care drug therapy in coordination with physicians, nurses, and other healthcare professionals
  • Actively participate in professional organizations to stay updated on industry standards and advancements
